Veterans Community Care in San Diego

Chiropractic, Acupuncture, Rehab, and Sports Injury Care for Veterans

At Peak Form Health Center, we are proud to support Veterans in San Diego through VA Community Care when care is authorized by the VA.

Our clinic provides chiropractic care, acupuncture, soft tissue therapy, rehab, and performance-based treatment for Veterans who want to move better, reduce pain, build confidence, and return to the activities that matter most.

Whether you are dealing with back pain, neck pain, headaches, joint pain, tendon pain, mobility limitations, or an old injury that keeps flaring up, our goal is to help you feel stronger, more capable, and less limited by pain.

What Is VA Community Care?

VA Community Care allows eligible Veterans to receive care from approved community providers when VA cannot provide the care needed, based on eligibility, availability, and the Veteran’s individual needs and circumstances.

For Veterans, this means you may be able to receive care at Peak Form Health Center if your VA care team authorizes the referral.

Community Care is not automatic. Veterans typically need a referral and authorization from their VA health care team before scheduling care with a community provider.

Services Available for Veterans at Peak Form

Peak Form Health Center offers several services that may be available through VA Community Care with proper authorization.

Chiropractic Care

Chiropractic care can help Veterans with back pain, neck pain, mobility limitations, joint pain, headaches related to neck tension, and musculoskeletal injuries.

VA states that chiropractic services are part of the standard Medical Benefits Package available to eligible Veterans, and access is typically by referral from a VA primary care or specialty provider.

Rehab and Movement-Based Care

At Peak Form, care does not stop at symptom relief. We also help Veterans improve strength, mobility, stability, and confidence with movement.

Your plan may include:

  • Mobility work
  • Strength exercises
  • Balance and coordination training
  • Return-to-activity planning
  • Pain education
  • Movement coaching
  • Home exercise support

Our goal is to help you build capacity so you can do more of what matters to you.

Soft Tissue Therapy

Soft tissue care may be used to address muscle, tendon, and fascial restrictions that contribute to pain, stiffness, or limited movement.

This may include provider-performed manual therapy, Active Release Technique, and other hands-on treatment strategies based on your needs.

Conditions We Commonly Help Veterans With

Veterans often come to Peak Form after trying multiple treatments without lasting success. We commonly help with:

  • Low back pain
  • Neck pain
  • Sciatica and radiating pain
  • Headaches
  • Shoulder pain
  • Hip pain
  • Knee pain
  • Foot and ankle pain
  • Plantar fasciitis
  • Achilles tendon pain
  • Tennis elbow
  • Golfer’s elbow
  • Muscle and tendon injuries
  • Mobility restrictions
  • Strength deficits
  • Chronic pain and recurring flare-ups

Our Approach: You Are Not Broken

At Peak Form Health Center, we believe the body is adaptable and resilient.

Pain can be frustrating, but it does not mean your body is fragile. Many Veterans have been told to avoid activity, protect their spine, or stop doing the things they enjoy. While rest may help in the short term, long-term improvement usually requires a better plan.

Our care focuses on helping you:

  • Understand what is contributing to your pain
  • Calm symptoms when needed
  • Improve mobility and strength
  • Build confidence with movement
  • Return to meaningful activity
  • Learn how to manage flare-ups
  • Reduce dependence on passive treatment alone

We combine hands-on care with active strategies so you have tools that carry over into daily life.

How to Request Care at Peak Form Through VA Community Care

Step 1: Talk to Your VA Provider

Ask your VA primary care provider or care team about a Community Care referral to Peak Form Health Center. This can be done via secure messaging, phone or in-person.

You can say:

“I would like to request a VA Community Care referral to Peak Form Health Center in San Diego.”

If you are requesting a specific service, be clear about what you need:

“I would like a referral for chiropractic care with Dr. Kevin Rose, DC at Peak Form Health Center.”

Or:

“I would like a referral for acupuncture with Annie Boyd, L.Ac or Angela Draper, L.Ac at Peak Form Health Center.”

Step 2: Confirm the Service Is Authorized

Each service usually needs to be authorized by the VA. If you want chiropractic care and acupuncture, ask whether both can be included in the referral or whether separate authorizations are needed. Most commonly you will oonly be able to do one type of service at a time (either chiropractic or acupuncture).

Your authorization may include details such as the approved provider, authorization number, type of care, approved visits, or authorized date range.  If you would like to see us but we are not listed, please ask the VA to change the provider. You can see any provider within the network, however the VA often offers only the closest to your home address.

Step 3: Call Peak Form to Schedule

Once your VA referral is approved, call Peak Form Health Center to schedule your first visit. The VA may offer to schedule your appointment for you; if you’d like to see us, please request they contact us to schedule.

Call: (619) 818-4306

What to Expect at Your First Visit

Your first visit is designed to understand your full picture, not just your symptoms.

We will review:

  • Your current pain or injury
  • How long it has been going on
  • What treatments you have tried
  • What activities are limited
  • Your movement and strength
  • Your goals
  • Your VA authorization details

From there, your provider will explain what they find and begin building a care plan around your needs.

Your plan may include chiropractic care, acupuncture, soft tissue therapy, rehab exercise, or a combination of services depending on what was authorized and what is appropriate for your condition.
